2. 2 SUPERVISOR, ATTENDANT and ENTRANT z MUST KNOW: y Hazards that may be faced during entry y Mode, signs or symptoms, and consequences of exposure

3. 3 DUTIES OF ENTRY SUPERVISOR z RESPONSIBLE FOR DETERMINING IF ACCEPTABLE ENTRY CONDITIONS ARE PRESENT z Verify that the appropriate responses have been made on the permit z Verify that all tests specified by the permit have been conducted z Verify that procedures and equipment specified by the permit are in place

4. 4 ENTRY SUPERVISOR cont. z ACCEPTABLE ENTRY CONDITIONS cont. z Verify that rescue service, if required, is available and that the means for summoning them are operable z Sign the permit indicating that entry may begin

5. 5 ENTRY SUPERVISOR cont. z ACCEPTABLE ENTRY CONDITIONS cont. z Ensure removal of unauthorized individuals who enter or who attempt to enter the permit space during entry operations

6. 6 ENTRY SUPERVISOR cont. z AUTHORIZE & OVERSEE ENTRY z Determine that entry operations remain consistent with terms of the entry permit and that acceptable entry conditions are maintained at intervals dictated by the hazards and operations performed within the space, and whenever responsibility for entry operations is transferred

7. 7 ENTRY SUPERVISOR cont. z TERMINATES ENTRY z When operations covered by permit are completed z When a condition that is not allowed under the permit arises in or near the space

8. 8 DUTIES OF ATTENDANTS z AN INDIVIDUAL STATIONED OUTSIDE ONE OR MORE PERMIT SPACES WHO MONITORS THE AUTHORIZED ENTRANTS AND PERFORMS ALL ATTENDANTS DUTIES ASSIGNED IN THE PERMIT SPACE PROGRAM z Must know hazards that may be faced

9. 9 ATTENDANT cont. z Remain outside the space unless relieved by another attendant z Know possible behavioral effects of hazards in entrants

10. 10 ATTENDANT cont. z Continuously maintain an accurate count of authorized entrants in the permit space and ensure that there is a means for identifying authorized entrants (ie. Rosters, tags)

11. 11 ATTENDANT cont. z Communicate with entrants to monitor their status z Alert entrants to evacuate if attendant detects: y prohibited condition y behavioral effects of hazards in entrants y dangerous situation outside space y That s/he can no longer perform as attendant

12. 12 ATTENDANT cont. z Summon rescue/emergency services as soon as its determined entrants may need assistance to escape

13. 13 ATTENDANT cont. z UNAUTHORIZED PERSONNEL z Warn them that they must stay away z If they have entered the space, advise them to leave immediately z Inform authorized entrants and supervisor if unauthorized personnel have entered the space

14. 14 AUTHORIZED ENTRANT z AN EMPLOYEE WHO IS AUTHORIZED BY THE EMPLOYER TO ENTER A PERMIT SPACE z Must know hazards that may be faced during entry, including signs, symptoms, and consequences of exposure z Properly use equipment (ie. Testing and monitoring, ventilating communication, rescue, etc.) d(4)

15. 15 AUTHORIZED ENTRANT cont . z Must communicate with the attendant as necessary to enable the attendant to monitor their status and to enable the attendant to alert entrants of the need to evacuate the space

16. 16 AUTHORIZED ENTRANT cont . z ENTRANT MUST ALERT ATTENDANT WHENEVER: z Entrant recognizes any warning sign or symptom of exposure to a dangerous situation z Entrant detects a prohibited condition

17. 17 AUTHORIZED ENTRANT cont . z ENTRANT MUST EVACUATE AS QUICKLY AS PRACTICAL WHENEVER: z Attendant or supervisor orders so z Entrant recognizes warning signs/symptoms of hazardous exposure z Entrant detects prohibited condition z Evacuation alarm is activated
