  2. Division of Safety & Hygiene Accident Analysis • ACCIDENT: • An unplanned event that interrupts the completion of an activity and that may include injury, illness, or property damage.

  3. Division of Safety & Hygiene Accident Analysis • NEAR MISS: • An unplanned event that interrupts the completion of an activity that directly involves the workers and may include product damage but no personal injury.

  4. Division of Safety & Hygiene Accident Analysis RECORDING ACCIDENTS AND NEAR MISSES • Keep them simple (near misses can be reported orally) • Clear Communication • Review for trends (like injuries, locations) • Goal should always be the prevention of further incidents

  8. Division of Safety Hygiene Accident Analysis (2) Material: • Was there equipment failure? • What caused it to fail? • Was machinery poorly designed? • Were hazardous substances involved? • Were they identified? • Should Personal Protective Equipment have been used?

  9. Division of Safety & Hygiene Accident Analysis (3) Environment: • What were the weather conditions? • Was poor housekeeping a problem? • Was noise a problem? • Was there adequate light? • Were toxic gases, dusts, fumes present?
